High Fidelity Wraparound (HFW)

What is High Fidelity Wraparound (HFW)?

High Fidelity Wraparound (HFW) is an evidence-based care coordination process that supports children and youth with serious behavioral health challenges. It helps them live successfully in their communities with their families. The approach focuses on strengths and cultural values of the youth, family, and support system to create individualized plans that address specific needs.

Responsibilities of an HFW Facilitator:

  • Engaging youth and families in the process.
     
  • Helping families identify their primary needs.
     
  • Preparing the team of service providers for the family.
     
  • Leading team meetings to develop an effective, integrated plan.
     
  • Monitoring and adjusting the plan to ensure ongoing effectiveness.
     
  • Regular communication with the youth, family, and team to assess the success of the plan.
     
  • Developing Strengths, Needs, and Culture Discovery for the family to identify priority needs.
     
  • Connecting the family with appropriate resources.
     

Requirements for HFW Facilitators:

  • A Bachelor’s degree or equivalent experience.
     
  • Broad life experience with diverse family needs.
     
  • Experience working with youth and families in different settings or personal experience with youth who have complex emotional needs.
     
  • A team player with an outgoing personality who can engage with people from various cultures, ages, and backgrounds.
     
  • Experience with systems like schools, mental health, child welfare, juvenile justice, and health services is beneficial.

Child and Youth Mentorship

What is Child and Youth Mentorship?

Child and Youth Mentorship supports children and youth by aligning strategies, interventions, and supports to prevent out-of-home placement. Mentors play a key role in maintaining stabilization, preventing crises, and de-escalating crises when necessary.

Responsibilities of a Child and Youth Mentor:

  • Communicate with administrative staff to ensure effective coordination and implementation of plans and goals.
     
  • Treat individuals with dignity and respect, creating a supportive environment.
     
  • Represent Pikes Peak Respite Services (PPRS) professionally to families, service coordinators, and community partners.
     
  • Build a positive rapport with the child/youth through consistent and enjoyable meetings.
     
  • Act as a cheerleader, coach, and advocate for the children, supporting their growth.
     
  • Provide a model for healthy and trusting relationships.
     
  • Recognize and encourage the strengths, talents, and gifts of the child/youth.
     
  • Listen actively and offer appropriate support and encouragement.
     
  • Engage in activities outside of the home to expose the child/youth to new opportunities.
     

Requirements for Mentors:

  • An updated resume highlighting relevant experience.
     
  • Broad life experience and a diverse perspective on the needs of families.
     
  • An outgoing personality that fosters engagement with individuals from different cultures, ages, and backgrounds.
     
  • A team player mentality for effective collaboration.

Respite Services

What is Respite?

Respite services provide relief to primary caregivers (non-paid supports) who care for individuals with disabilities. It offers short-term care for individuals, allowing caregivers a break.

Individual Respite:

  • One-on-one care for 10 hours or less within a 24-hour period.
     
  • Provided in the participant's home or community.
     

Group Respite:

  • Typically for siblings, negotiated directly by the provider with the family.
     
  • Day Respite available only for contractors.
     

Job Type: Contract/Employee

Youth Day Services (Individual or Group)

What are Youth Day Services?

Youth Day Services support children and youth during work hours, offering temporary relief to primary caregivers.

  • Individual Youth Day: Up to 10 hours in a 24-hour period.
     
  • Group Youth Day: Available with prior coordinator and agency approval.

Supportive Community Connector (SCC)

What is a Supportive Community Connector?

The SCC helps individuals gain the skills necessary to access community activities and supports socialization and community involvement.

Responsibilities of an SCC:

  • Supports individuals in accessing social activities, building friendships, and maintaining relationships.
     
  • Facilitates engagement in community groups, education, or training.
     
  • Encourages participation in volunteering and community events.
     

SCC Transport Reimbursement:

  • $10 per shift, limited to 20 miles round trip.
     
  • Effective March 2024.

Community Connector

What is a Community Connector?

Community Connectors help individuals engage in activities that build relationships and natural supports within the community.

Responsibilities:

  • Facilitates access to socialization, skill-building, and community participation.
     
  • Provides services as outlined in the individual’s service plan.
     

Community Connector Transport Reimbursement:

  • $8 per shift, no claim required in Sirius Care.
     
  • Limited to 20 miles round trip.
     
  • Effective March 2024.

Mentorship Services

What is Mentorship?

Mentorship promotes self-advocacy and independence through various methods such as instructing, modeling, advising, and providing experiences.

Homemaker Basic / Homemaker Services

What is Homemaker Basic?

Homemaker services help with basic household tasks, such as cleaning, laundry, and maintenance, due to the participant’s disability. This service supports the primary caregiver in managing the home and care for the participant.

Homemaker Enhanced Services

What is Homemaker Enhanced?

In addition to basic homemaker tasks, Enhanced Homemaker services include habilitation and extraordinary cleaning due to behavioral or medical needs.

Responsibilities:

  • Habilitation: Training the participant to perform tasks independently.
     
  • Extraordinary Cleaning: Includes cleaning related to medical or behavioral needs.

Non-Medical / Personal Care Services

What is Non-Medical / Personal Care?

Personal care workers assist with daily activities such as:

  • Observing and maintaining a safe home environment.
     
  • Household chores (cooking, cleaning, laundry).
     
  • Assisting with appointments or shopping.
     
  • Offering companionship and social interaction.
     
  • Helping with activities of daily living and personal care tasks.
