High Fidelity Wraparound

 

What is High Fidelity Wraparound?


  • High Fidelity Wraparound (HFW) is an evidence-based care coordination process that effectively supports children and youth with serious behavioral health challenges to live successfully in their communities with their families. It is a strength-based approach that identifies and builds upon the strengths and culture of the youth, family, and support system to create integrated and individualized plans to address their needs.


  • The responsibilities of a HFW facilitator include working with support partners to engage the youth and family in the process, helping the family identify and articulate their primary needs, and preparing the team of service providers and supports for the family. The facilitator also leads team meetings to develop an effective and integrated plan that focuses on the identified needs, monitors the implementation of the plan to ensure ongoing effectiveness, and communicates frequently with the youth, family, and team to assess the success of the HFW process.


  • A HFW facilitator plays a crucial role in assessing the level of self-efficacy of the family and determining the success of the process by how well it builds confidence and skills for the youth and family. They also develop the Strengths, Needs, and Culture Discovery for the family, which defines the priority needs, and connect the family to the appropriate resources while ensuring timely implementation of all aspects of the plan.


  • Requirements for a HFW facilitator include a Bachelor's degree or equivalent experience, an updated Curriculum Vitae, and broad life experience with a diverse view of what families need to have better lives. It is important to have direct experience working with youth and their families in various settings or lived experience with youth who have complex emotional needs. An outgoing personality that supports engagement with people from different cultures, ages, and backgrounds is preferred. Additionally, understanding and experience with different systems, such as schools, mental health, child welfare, juvenile justice, health, and others, is beneficial. Being a team player is also essential in effectively carrying out the responsibilities of a HFW facilitator.

Child and Youth Mentor

 

Child and Youth Mentorship is an important component of the wraparound process, aimed at aligning strategies, interventions, and supports for children and youth and their families to prevent the need for out-of-home placement. Mentors play a key role in maintaining stabilization, preventing crisis situations, and de-escalating crises when they occur.


Responsibilities of a Child and Youth Mentor include:


1. Communicating with administrative staff to ensure effective coordination and implementation of individual plans and behavioral goals.


2. Treating individuals with dignity and respect, fostering a positive and supportive environment.


3. Representing Family Pikes Peak Respite Services professionally to families, service coordinators, and community partners.


4. Developing a personal and positive rapport with the child or youth through consistent and enjoyable meetings, creating a comfortable and trusting relationship.


5. Acting as a cheerleader, coach, and advocate for the children, supporting and encouraging their growth and development.


6. Providing a model for a healthy and trusting relationship by practicing clear communication and setting appropriate boundaries.


7. Recognizing and acknowledging the strengths, talents, and gifts of the child or youth, and encouraging them to utilize and explore these qualities.


8. Actively listening to the child or youth, offering support and encouragement when appropriate.


9. Engaging in activities outside of the home to expose the child or youth to new opportunities and experiences.


Requirements for a Child and Youth Mentor include:


1. An updated resume highlighting relevant experience.


2. Broad life experience and a diverse perspective on the needs of families to support their well-being.


3. An outgoing personality that fosters engagement with individuals from different cultures, ages, and backgrounds.


4. A team player mentality, collaborating effectively with others to achieve positive outcomes.


Child and Youth Mentorship plays a vital role in the wraparound process, providing individualized support and guidance to children and youth. Through their positive relationships and advocacy, mentors contribute to the overall success and well-being of the individuals they serve and help create a nurturing environment for their growth and development.


Job Type: Contract/Employee

Contractor Pay Rate: $23.00/hour

Employee: $21.25/hour



Host Homes 


Host Homes are a compassionate and personalized support program where individuals with intellectual and developmental disabilities are welcomed into the homes of caring Host Home Providers. The program focuses on creating meaningful and long-term relationships through a highly personalized matching process that prioritizes personal choice.


Responsibilities of a Host Home Provider include:


1. 24-hour home-based support: Providing a safe and supportive environment for the individual, ensuring their well-being and meeting their daily needs.


2. Whole life planning: Collaborating with the individual to develop a comprehensive plan that encompasses their goals, aspirations, and long-term plans.


3. Life skills development: Assisting the individual in developing and enhancing essential life skills such as communication, self-care, and social interaction.


4. Activities of daily living management: Supporting the individual with tasks related to daily living, including personal hygiene, meal preparation, and household chores.


5. Family support: Engaging with the individual's family members and providing support, guidance, and collaboration as needed.


6. Transportation: Assisting the individual in accessing community resources, appointments, and activities by providing transportation.


7. Medication management oversight: Ensuring that the individual's medication is administered correctly and following prescribed guidelines.


8. Individual service planning: Collaborating with the individual, their support team, and service providers to develop and implement an individualized service plan.


9. Structured activities: Engaging the individual in structured activities that promote personal growth, skill development, and community integration.


10. Community integration: Supporting the individual in actively participating in their community, fostering social connections, and accessing community resources and activities.


11. Advocacy services: Advocating for the individual's rights, needs, and preferences within the community and the service system.


12. 24-hour on-call support: Being available to respond to any emergencies or concerns that may arise outside of regular hours.


13. Respite: Providing respite care for the individual, allowing their primary caregivers to have a break and rest.


Requirements for becoming a Host Home Provider include:


1. A passed home inspection that meets or exceeds the state requirements, conducted by a member of the PPRS Admin Team. This ensures that the home provides a safe and suitable environment for the individual.


Host Homes offer a nurturing and inclusive living arrangement where individuals with intellectual and developmental disabilities can thrive. Host Home Providers play a crucial role in providing personalized support, fostering independence, and creating a sense of belonging for the individuals they welcome into their homes.

  IRSS Host Home Level One -   $52.50/day
 IRSS Host Home Level Two -  $84.00/day
 IRSS Host Home Level Three -  $105.00/day
 IRSS Host Home Level Four -  $127.50/day
 IRSS Host Home Level Five -  $150.00/day
 IRSS Host Home Level Six -  $200.00/day

  IRSS Host Home Level Seven -  $210.00/day

Job Type: Contract


  

Specialized Habilitation Services (Adults 18 and older only)


  • Help adults with tasks associated with maximum functionality and self-sufficiency such as Self-feeding, Toileting, Self-Care, maintenance skills. Can be lessons taught in school, and therapy where the provider reinforces those lessons.
  • 1 on 1 service
  • Service is reimbursed based on the SIS level of the person served

Pay Rates: 


SH Level 1-5: IC: $18.00 hourly, Employee: $17.00 hourly

SH Level 6:  IC: $21.52 hourly, Employee: $20.00 hourly

SH Level 7:  IC: $27.00 hourly, Employee: $25.00 hourly

Individual: IC: $17.00 

Supported Employment Services 


What is Supported Employment? 


  • Service provided in order to enable waiver participants to gain access to waiver and other community services, activities, and resources, as specified by the service plan.
  • Transportation to and from work us a benefit in conjunction with supported employment service except when the supported employment service occurs at a frequency less than the number of days worked. In that case, transportation to and from the place of employment is a benefit when participant does not have resources available, including:
  • Personal Funds
  • Natural Supports
  • Third-party resources
  • This service is offered in addition to medical transportation required and transportation services under the state plan.
  • Transportation services under the waiver are offered in accordance with the participants service plan.
  • Whenever possible, family, neighbors, friends, or community agencies, which can provide this service without charge, are utilized.
  • Transportations to and from day program shall be reimbursed based on the applicable transportation band.



Respite Services 


What is Respite? 


  • Respite Services is an individually designed service intended to provide relief to natural (informal), non-paid supports who provide primary care and support to a waiver participant.
  • For individual respite, the client receives respite in a one-on-one situation. There are no other clients in the setting also receiving respite services. Individual respite occurs for 10 hours or less in a 24-hour period.
  • Respite may be provided on an individual or group basis in the residence of the participant or respite care provider (upon completion of a passed home inspection conducted by a member of the PPRS Admin Team) or in the community. 
  • Respite service is provided on a short-term basis, because of the absence or need for relied to caregivers of the participant. Respite is to be provided in an age-appropriate manner.


Pay Rates: 

Job Type: Contract (Rates beginning July 1, 2023) 

  • For Regular Respite Shifts (10 hours or less) $19.28/hour

Group Respite 

  • IC: $19.28 per hour per waivered person- Typical sibling’s rates are negotiated directly by the provider with the family
  • For Day Respite Shifts (10 hours or more) $225.00/day (Contractors Only)


Job Type: Employee (Rates beginning July 1, 2023) 

  • For Regular Respite Shifts (10 hours or less) $18.00/hour

 Group Respite: 

  • Employee: $18.00 per hour per waivered person- Typical sibling’s rates are negotiated directly by the provider with the family.




Supportive Community Connector 


What is a Supportive Community Connector? 


Supported community connections supports the abilities and skills necessary for individuals to access typical activities and community life. We emphasize helping individuals to get involved in their community by connecting to resources and supporting them in their efforts.


This includes :

  • Socialization
  • Meet people who have similar interests and hobbies
  • Find and maintain healthy friendships and natural supports
  • Access community education or training
  • Join and participate in community groups and committees
  • Volunteer in your community
  • Responsibilities of a Supportive Community Connector
  • Supports the abilities and skills necessary to enable the individual to access typical activities and functions of community life such as those chosen by the population.
  • Provides a wide variety of opportunities to facilitate and build relationships and natural supports in the community while utilizing the community as a learning environment to provide services and supports as identified in the participants service plan.
  • Provides a wide variety of opportunities to facilitate and build relationships and natural supports in the community while utilizing the community as a learning environment to provide services and supports as identified in the participants service plan.


Pay Rates:


SCC 1 through 3 - IC: $15.76 per hour, Employee: $15.75 per hour

SCC 4- IC: $17.00 per hour, Employee: $16.00 per hour

SCC 5- IC: $18.00 per hour, Employee: $17.00 per hour

SCC 6- IC: $21.00 per hour, Employee: $19.50 per hour

SCC 7- IC: $27.00 per hour, Employee: $25.00 per hour

SCC 7- Pueblo RCF $25 per hour IC and Employee


Non-Medical Transportation: (2 claims per SCC shift, for SCC only)

- Travel reimbursement for driving on SCC shifts.

- 1 claim for driving your client to the community before your SCC shift starts.

- 1 claim after the SCC shift ends and you’re driving your client home.

IC and Employee: MB1- $8.00 per one-way trip (10 miles or less per one-way trip)

IC and Employee: MB2- $11.50 per one-way trip (Between 10 miles and 20 miles per

one way trip)

IC and Employee: MB3- $17.00 per one-way trip (Between 20 miles and 30 miles per

Community Connector 


What is a Community Connector?


  • Community Connectors provide a wide variety of opportunities to facilitate and build relationships and natural supports in the community while utilizing the community as a learning environment to provide services and supports as identified in the participants service plan.
  • These types of services may include –
  • Socialization 
  • Adaptive Skills 
  • Personnel to accompany and support the individual in community settings.
  • Participation in activities and supplies related to skill acquisition, retention, or improvement. 
  • Responsibilities 
  • Supports the abilities and skills necessary to enable the individual to access typical activities and functions of community life such as those chosen by the population.
  • Provide a wide variety of opportunities to facilitate and build relationships and natural supports in the community while utilizing the community as a learning environment to provide services and supports as identified in the participants service plan.
  • Provide a wide variety of opportunities to facilitate and build relationships and natural supports in the community while utilizing the community as a learning environment to provide services and supports as identified in the participants service plan.

Job Type: Employee - (Rates after July 1, 2023)

Employee Mileage for CC- $8.00 per shift (1 claim per shift)


Mentorship Services 


What is Mentorship?

 

  • This service is provided to participants to promote self-advocacy through methods such as:
  • Instructing 
  • Providing Experiences 
  • Modeling 
  • Advising
  • Mentorship promotes independence with skills around more formal activities in the community.


Pay Rates:

Job Type: Employee - (Rates after July 1, 2023)

Pay Rate: $25.00/hour


Homemaker Basic


What is Homemaker Basic? 


  • Services that consist of the performance of basic household tasks within the participant’s primary residence, such as:
  • Cleaning
  • Laundry
  • Household Care
  • Maintenance which is related to the participant’s disability and provided by a qualified homemaker, when the parent or primary caretaker is unable to manage the home and care for the participant in the home.
  • Any assistance must be due to the participants disability that results in additional household tasks and increases the parent/caregiver’s ability to provide care needed by the participant.
  • Assistance may take the form of hands-on-assistance or cuing to prompt the participant to perform a task. 

  • Pay Rates:


Job Type: Contract -  (Rates after July 1, 2023)

Pay Rate: $17.00./hour

Job Type: Employee -  (Rates after July 1, 2023)

Pay Rate: $15.75/hour


Homemaker Enhanced 


What is Homemakers Enhanced? 


  • Services provided by a qualified homemaker that consists of the same household tasks as described under Basic Homemaker Services with the addition of either habilitation or extraordinary cleaning.
  • Habilitation includes direct training and instruction to the participant, which is more than basic cuing to prompt the participant to perform a task.
  • Habilitation shall include a training program with specific objectives and anticipated outcomes.
  • There may be some amount of incidental basic homemaker services that is provided in combination with enhanced homemaker services, however, the primary intent must be it provide habilitative services to increase the independence of the participant.
  • Habilitation may include some hands-on assistance or cuing to prompt the participant to perform a task, only when such support is incidental to the habilitative services being produced and the primary duties must be to provide habilitative services to increase independence of the participant.
  • Enhanced homemaker services also include the need for extraordinary cleaning as result of the participants behavioral or medical needs.

Includes: 

  • Same household tasks as described under homemaker basic with addition
  • of teaching the tasks or extraordinary cleaning.
  • Goal based service- must fill out proper paperwork and have an understanding of the service plan.
  • Primary goal is to increase independence where applicable.
  • Extraordinary cleaning is a result of behavioral or medical needs, such as need for dust-free environment due to allergies or cleaning of fecal smearing due to behaviors.


Does Not Include:


  • Animal Care
  • Homemaker are not responsible for aquariums, terrariums, cages, litter boxes, kennels, outside yard area, etc. 
  • Service animals are not an exception.
  • Toy/Book Organization
  • Toys and books can be gathered to one area for floor care, such as vacuuming and sweeping.
  • Personal Care Services
  • Moving Furniture or Appliances.
  • Climbing to Clean
  • Yardwork
  • Putting up Holiday Decorations
  • Pool Area

Non-Medical / Personal Care


What is Non-Medical / Personal Care? 


  • The duties of a personal care worker may include the following – 
  • Observation and maintenance of the home environment that ensures the safety and security of the consumer. 
  • Assistance with household chores including cooking and meal preparation, cleaning, and laundry. 
  • Assistance in completing activities such as shopping, and appointments outside the home. 
  • Companionship including, but not limited to, social interaction, conversation, emotional reassurance, encouragement of reading, writing and activities that stimulate the mind.
  • Assistance with activities of daily living, personal care and any other assignments as included in the service plan.
